To notify us of a claim,

Call our Annuities Department at (800) 233-3750 ext. 510 and have the following information available:

  • Name(s) of deceased
  • Contract number(s)
  • Date of death
  • Date of birth
  • Your contact information and relationship to the deceased.

Once we are notified, we will send forms and instructions to file your claim.
